Terry Ann Kwiatkowski, 64, of Butler passed away Feb. 17 at her residence after a prolonged illness.

She was born June 16, 1953, in Pittsburgh, and was the daughter of the late Gerald Robert and Frances (Panko) Phillips.

Surviving are her husband of 36 years, Terence W. Kwiatkowski of Butler; two sons, Lee (Chrissie) Koss of Kersey; Brian (Magda) Koss of Mohrsville; six grandsons, Corey, Nathan, Ben, Jacob, Brian H. and Thomas Koss; several nieces, nephews, great-nieces and great-nephews; and a sister she never had but always wanted, Lillian Meyers of Evans City.
