John R. Kolakowski, 29, of Butler passed away Monday.

Born Sept. 6, 1986, in Pittsburgh, he was a son of Bridgid L. (Gross) Kolakowski and the late Robert J. Kolakowski.

He was a graduate of Butler High School.

John is survived by his mother, Bridgid Kolakowski, and his sister, Shannon Kolakowski, both of Cincinnati, Ohio; and a number of aunts, uncles and cousins.

He was preceded in death by his father, Robert Kolakowski, and his grandparents.
